The following form is required for any ERWSD account to allow an individual or management firm the ability to access account information or receive email communications relevant to the account. This could include emergency water use notifications, email leak alerts, high use notifications, bill copies, or other account related communications. By completing this form you are acknowledging that you or your organization has a specified contract allowing for access to billing and water consumption information.
